The Indigenous Health Program which is a key component of the Australian Government’s “Closing the Gap” initiative, aims to support general practices and Indigenous health services in providing better health care for Aboriginal and/or Torres Strait Islander patients.

Health Professionals in General Practice provide Indigenous Australians access to adequate, preventive and comprehensive primary health care essential to close the gaps in early childhood mortality and life expectancy. Closing the Gap initiatives focus on prevention, including the promotion of healthy lifestyles at all ages and the related management and treatment of chronic diseases among Aboriginal and/or Torres Strait Islander patients.

Doctors at the practice are able to conduct Aboriginal and/or Torres Strait Islander health checks which is the first step to make sure these patients achieve the best level of health care as it encourages early detection, diagnosis and intervention for treatable conditions and best practice management of chronic disease.  Aboriginal and/or Torres Strait Islander patients with a chronic medical condition are also eligible for a GP Management Plan and the benefits of Team Care arrangements to better manage their health care needs and wellbeing.
