Transfer of Medical Records

We understand that patient health information is required for many different reasons to ensure that the appropriate care is provided.

To ensure we are able to supply the appropriate information to other services or for us to receive the appropriate information we always require a signed consent to release information form to be used (signed by the patient or legal guardian).

When we are providing information to another medical service we will supply a patient health summary free of charge as a professional courtesy.  This summary will not included any third party results or reports.  The third parties will need to be contacted directly to request the information from their service.

If more information that what is contained in the summary is required there will be an administration fee charged for the report and the information will not be released until payment is received.

If a patient is requesting their own information an administration fee will be charged for the patient health summary.

There are also fees payable for request for information from lawyers and insurance companies and again, information will not be released until payment has been received.

Please note we can not accept a normal email request that does not include a signed request for information letter or form.
